EMS103 – Provision Request

This form is for schools to request a new music provision provided by Ealing Music Service.

Complete one form per provision e.g. if schools wish to request two different provisions, two different requests must be submitted.

EMS teachers can also use this form to claim for ad hoc EMS activities funded by Ealing Music Service.

Once received, Lee Marchant, Deputy Head of Service will be in touch to discuss the new request(s).
