EMS103 – New or Ad-Hoc Provision Request

This form is for schools to request a New or Ad-Hoc Music Provision provided by Ealing Music Service.

EMS teachers must complete the EMS Teacher Ad-Hoc Pay Claim form to claim for ad hoc EMS activities.

• Complete one form per provision e.g. if schools wish to request two different provisions, two different requests must be submitted.
• New Provision requests can be made for a wide range of instruments and singing styles for Individual and Small-Group Tuition and Ensemble tuition. e.g. Small Group Violin.
• Ad-Hoc Provision requests can be made to ensure an EMS teacher is paid for ad-hoc work that they have complete at your school. e.g. Leading an assembly, an extra lesson outside of pre-agreed tuition. These ad-hoc provisions will be included on your next school billing.
• All requests will be reviewed and approved by a member of EMS SLT.
